Liability Waiver, Release of Claims, Assumption of Risk, Code of Conduct, Hygiene Policy & Photo/Video Consent

1. Assumption of Risk
   I, the undersigned Participant (or Parent/Legal Guardian if the Participant is a minor), acknowledge and understand
   that participation in Brazilian Jiu Jitsu, grappling, martial arts instruction, conditioning, sparring, self-defense training,
   competitions, open mat sessions, seminars, demonstrations, and all related activities (collectively, the “Activities”)
   offered by Praxis Jiu Jitsu Academy LLC (“the Academy”) involves inherent and significant risks. These risks include,
   but are not limited to, bruises, strains, sprains, joint injuries, fractures, dislocations, concussions, paralysis, permanent
   disability, death, illness, infection, and property damage. I understand that these risks may arise from my own actions
   or inactions, the actions or inactions of other participants, the condition of the premises or equipment, or the ordinary
   negligence of the Academy. I knowingly, voluntarily, and expressly assume all such risks, whether known or unknown.
2. Acknowledgment of Physical Condition
   I represent that I am physically fit and capable of participating in the Activities and that I have no medical condition
   that would make participation unsafe. I understand that Praxis Jiu Jitsu Academy LLC does not provide medical
   insurance or medical care and that I am solely responsible for all medical expenses incurred as a result of
   participation.

3. Waiver and Release of Liability
In consideration for being permitted to participate in the Activities, I, on behalf of myself, my heirs, executors,
administrators, legal representatives, and assigns, hereby release, waive, discharge, and covenant not to sue Praxis
Jiu Jitsu Academy LLC, its owners, members, managers, officers, instructors, employees, contractors, volunteers,
agents, and affiliates (collectively, the “Released Parties”) from any and all claims, demands, causes of action,
damages, losses, or liabilities of any kind arising out of or related to my participation in the Activities, including claims
arising from the ordinary negligence of the Released Parties, to the fullest extent permitted under the laws of the State
of New Mexico. This waiver does not apply to gross negligence or intentional misconduct where prohibited by law.

4. Indemnification
I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, damages, losses,
costs, or expenses, including reasonable attorneys’ fees, arising out of or related to my participation in the Activities or
my violation of this agreement.

5. Emergency Medical Authorization
I authorize Praxis Jiu Jitsu Academy LLC to obtain emergency medical treatment on my behalf if Academy staff
reasonably believes such treatment is necessary. I understand and agree that I am financially responsible for all
medical treatment and related costs.

6. Photo and Video Release
I grant Praxis Jiu Jitsu Academy LLC the irrevocable, perpetual, worldwide, royalty-free right to photograph, video
record, and otherwise capture my image, likeness, voice, or performance during participation in the Activities. I
authorize the Academy to use such media for lawful business purposes, including marketing, advertising, promotional
materials, social media, and website content, without compensation.

7. Code of Conduct
I agree to conduct myself in a safe, respectful, and professional manner at all times while on Academy premises or
participating in Academy-related activities. This includes following instructor directions at all times, training within my
ability level, immediately tapping and releasing submissions, refraining from striking unless instructed, not coaching
unless authorized, avoiding harassment or intimidation, not training under the influence of drugs or alcohol, and not
engaging in unsafe or disruptive behavior. The Academy reserves the right to suspend or terminate participation or
membership for violations of this Code of Conduct.

8. Hygiene and Health Policy
I acknowledge that Brazilian Jiu Jitsu is a close-contact activity and agree that proper hygiene is mandatory. I agree to
arrive clean, wear clean training attire for every session, wash all gis, rashguards, belts, and training clothing after
each use, keep fingernails and toenails trimmed, secure long hair, cover all cuts, wear footwear off the mats, and not
train with open wounds, skin infections, contagious illness, fever, or flu-like symptoms. The Academy may require me
to leave training if a hygiene or health concern is suspected.

9. Minors
If the Participant is under eighteen (18) years of age, I certify that I am the parent or legal guardian and have legal
authority to sign this agreement. I consent to the minor’s participation and agree to all terms of this agreement on
behalf of myself and the minor.

10. Severability
If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full
force and effect.

11. Governing Law and Venue
This agreement shall be governed by and interpreted in accordance with the laws of the State of New Mexico, and
any legal action shall be brought in a court of competent jurisdiction within the State of New Mexico.
I HAVE READ THIS ENTIRE AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I UNDERSTAND THAT
BY SIGNING THIS DOCUMENT, I AM GIVING UP CERTAIN LEGAL RIGHTS.
